Cesarean scar pregnancy is a specific and uncommon form of ectopic pregnancy in which implantation occurs within the scar tissue of a prior C-section. Although rare, this possibility is one reason women with multiple surgical deliveries are often monitored carefully in early pregnancy.

It is important to understand that none of these locations can safely support a growing pregnancy. As the embryo grows, surrounding tissue can become damaged, which is why early recognition and medical supervision are essential.

How Is It Different From a Chemical (Biochemical) Pregnancy?

A biochemical pregnancy occurs when implantation likely begins inside the uterus but stops developing very early — often before anything can be seen on ultrasound.

An ectopic pregnancy, by contrast, involves implantation in the wrong location. The pregnancy may continue to produce hCG and even appear to progress temporarily, but it is not viable and can become dangerous if not identified.

Both situations can involve rising or fluctuating hCG levels and early uncertainty, which is why careful monitoring is so important in the first weeks of pregnancy.

This foundational understanding matters because many early pregnancies present with similar initial signs — mild bleeding, slow-rising hCG levels, or inconclusive ultrasound findings. Distinguishing between them requires patience, structured follow-up, and clinical evaluation.

How Common Is It — and Does Age Increase the Risk?

Ectopic pregnancy is relatively uncommon, but not rare. It is estimated to occur in about 1–2% of all reported pregnancies.

While the overall percentage is low, certain factors increase the likelihood, and age is one of them.

Does Age Matter?

The risk of ectopic pregnancy gradually increases after the age of 35 and rises further after 40.

This does not mean most pregnancies after 35 will be ectopic — far from it. But biologically, several age-related changes can influence risk:

  • Subtle changes in fallopian tube function
  • Higher rates of prior pelvic infections over time

Procedures such as IVF slightly increase the risk of ectopic pregnancy. Even when embryos are transferred directly into the uterus, implantation outside the ideal location can still occur.

In rare cases, implantation may occur within the scar tissue of a previous C-section. This form of ectopic pregnancy is uncommon but clinically significant, and it highlights how surgical history can become relevant in early pregnancy evaluation.

It is important to understand that most ectopic pregnancies are not caused by something a woman did or failed to do.

Often, the underlying reason cannot be identified with certainty.

Biology is complex. Even in healthy women with no known risk factors, ectopic pregnancy can occur. The presence of risk factors increases probability — but it does not create inevitability.

In a typical early pregnancy, hCG levels roughly double every 48 hours. This pattern helps clinicians assess whether a pregnancy is progressing normally.

In ectopic pregnancy, hCG levels often rise more slowly, plateau, or fluctuate unpredictably. However, patterns are not always straightforward. Some ectopic pregnancies may initially appear to rise within expected ranges, which is why hCG values alone cannot confirm or exclude the diagnosis.

In my own experience, hCG levels continued to rise — but not appropriately. They increased from one blood draw to the next, yet never doubled as expected. For nearly a month, I had blood tests every 48 hours and underwent multiple ultrasounds. Nothing definitive appeared on imaging, yet the hormone levels did not decline.

This period is often described medically as a “pregnancy of unknown location.” It is a state of diagnostic uncertainty where careful monitoring is essential, particularly because ectopic pregnancy must be ruled out before assuming a loss is resolving naturally.

Repeated testing can be physically and emotionally draining. I remember one moment clearly — after weeks of blood draws, my veins were so exhausted that during one appointment, blood would barely flow from my arm.
